It's dropped out of fashion because of cultural awareness of health etc., but smoking used to be an incredible way to highlight tension in a movie. Watch any movie with Humphrey Bogart...the point at which he pulls out a cigarette is always the point where tension ratchets up.
The film studios were also heavily funded by cigarette companies who had no restrictions on advertising and product placement back then. Smoking was encouraged by doctors then as a way to relax. Times changed.
